An outstanding opportunity awaits for an experienced Home Health Physical Therapist to deliver compassionate, in-home patient care. This contract role calls for a dedicated clinician comfortable working independently in the field, conducting comprehensive assessments, developing patient-centered therapy plans, and facilitating meaningful recovery for patients within their home environments.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active Physical Therapist (PT) license in good standing
• At least 1 year of recent home health PT experience (field/home visit experience strongly preferred)
• Proficiency with Home Care Home Base (HCHB) documentation system preferred
• OASIS assessment experience required
• Excellent evaluation, clinical judgment, and treatment planning skills
• Reliable transportation, valid drivers license, and current auto insurance
• Strong communication, organizational, and time management abilities

Primary Responsibilities:

• Conduct thorough initial and ongoing physical therapy evaluations for home health patients, including OASIS assessments as applicable
• Design and implement individualized care plans supporting mobility, strength, balance, pain management, and patient safety
• Administer therapeutic interventions such as exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies
• Educate patients and caregivers on exercises, use of assistive devices, and injury prevention techniques in the home setting
• Reassess progress, adjust care plans when warranted, and coordinate closely with interdisciplinary team members
• Document services, progress, and communication accurately within Home Care Home Base (HCHB)
• Engage in case conferences and contribute to a collaborative, patient-centered care approach

$2,602 - $2,753/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
