Job Description

Deliver skilled physical therapy services directly in patients homes as a Home Health Physical Therapist Assistant (PTA). This role involves implementing individualized treatment plans to enhance mobility, strength, balance, and overall functional independence under the supervision of a licensed Physical Therapist. Ideal candidates are organized, self-motivated, and comfortable managing a field-based schedule while prioritizing patient-centered care.

Key Responsibilities:

  • Administer skilled physical therapy treatments aligned with the established plan of care.
  • Assist patients with therapeutic exercises, gait and balance training, transfer techniques, and functional mobility.
  • Monitor and document patient progress and responses to treatments.
  • Report any changes in patient condition to the supervising Physical Therapist and healthcare team.
  • Educate patients and caregivers on safety strategies, assistive device use, fall prevention, and home exercise programs.
  • Maintain accurate and timely electronic documentation using Home Care Home Base (HCHB).
  • Collaborate with an interdisciplinary healthcare team to ensure comprehensive patient care.
  • Participate in case conferences, team meetings, and adhere to regulatory and Medicare home health guidelines.

Qualifications:

  • Active Physical Therapist Assistant (PTA) license in good standing within the state of practice.
  • Minimum of 1 year recent Home Health PTA experience, preferably field-based.
  • Proficiency with Home Care Home Base (HCHB) and familiarity with OASIS documentation strongly preferred.
  • Strong clinical observation, communication, and organizational skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently and manage a flexible schedule in a field-based environment.

Schedule & Location:

  • Full-time, 13-week contract.
  • Monday to Friday, 8:00 AM 5:00 PM.
  • Serving patients within a designated territory in Benton City, WA, with daily travel (mileage reimbursement provided).
