Bring your clinical expertise to a rewarding role as a Home Health Physical Therapist, where youll make a meaningful difference for patients in their home environments. In this contract opportunity, youll be a vital partner in restoring and maintaining each patients independence through comprehensive, evidence-based therapy.

Schedule & Setting:

• 13-week contract, full-time
• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points per week (57 patient visits per day, based on visit type)
• Field-based, travel within a designated territory (mileage reimbursement included)
• Utilizes the Home Care Home Base (HCHB) system and OASIS for assessments and documentation

Key Responsibilities:

• Perform in-depth physical therapy evaluations, including use of OASIS for care activation and transition points
• Develop, implement, and revise tailored treatment plans to address mobility, strength, and safety
• Deliver interventions such as gait and transfer training, neuromuscular re-education, and pain management
• Monitor and document patient progress, updating treatment plans as medically appropriate
• Educate patients and caregivers on rehabilitation exercises, safety tactics, and adaptive equipment use
• Collaborate with a multidisciplinary care team to coordinate care and attend case conferences
• Uphold compliance with agency protocols, regulatory requirements, and Medicare guidelines

Qualifications:

• Active, unrestricted Physical Therapist license in the state
• At least one year of recent field-based Home Health PT experience
• Proficiency with HCHB and required OASIS assessment experience
• Excellent clinical judgment, communication, and self-management skills
• Reliable transportation, drivers license, and auto insurance
