Make a significant impact on patient recovery and independence as a Home Health Physical Therapist providing care in a field-based setting. This full-time, 13-week contract position focuses on delivering high-quality, individualized physical therapy services directly to patients homes. Leverage your clinical knowledge and compassionate approach to promote mobility and enhance the quality of life for those in your care.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• Field-based position with daily travel to patient homes (mileage reimbursement provided)
• Expected caseload: 2532 points per week (typically 57 patient visits per day, depending on visit types)

Key Responsibilities:

• Complete thorough physical therapy evaluations, including OASIS assessments (Start of Care, Resumption, Recertification, and Discharge as needed)
• Develop and execute individualized treatment plans aimed at increasing functional mobility, strength, balance, and safety
• Deliver skilled interventions: therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention strategies
• Assess patient progress, update plans of care, and communicate with physicians and interdisciplinary team members
• Educate patients and caregivers on exercises, safety techniques, and the use of assistive devices
• Maintain accurate, timely documentation using Home Care Home Base (HCHB)
• Participate in case conferences, interdisciplinary meetings, and ongoing education

Qualifications:

• Current Physical Therapist (PT) license in Pennsylvania (active/good standing)
• Minimum 1 year recent home health physical therapy experience (field-based strongly preferred)
• Experience with OASIS and proficiency in Home Care Home Base (HCHB) desired
• Excellent assessment, critical thinking, and organizational skills
• Reliable transportation, valid drivers license, and current auto insurance

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
