Deliver impactful patient care directly in the comfort of patients homes as a Home Health Physical Therapist on a 13-week contract assignment. This vital role empowers you to help individuals regain independence and optimize their quality of life by implementing tailored physical therapy interventions throughout your assigned territory each day.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• Full-time, 13-week assignment
• Daily travel to patient homes (mileage reimbursement provided)
• Caseload: About 2532 points weekly, typically 57 patient visits per day

Key Responsibilities:

• Perform in-depth physical therapy evaluations and conduct OASIS assessments for Start of Care, Resumption, Recertification, and Discharge as needed
• Develop and implement therapy plans addressing functional mobility, strength, balance, gait, and safety
• Deliver skilled interventions including therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies
• Evaluate progress and adjust plans as necessary while ensuring clear communication with physicians and care teams
• Educate patients and families on exercise routines, precautions, and the correct use of assistive devices
• Maintain accurate, timely records through Home Care Home Base (HCHB)
• Participate in interdisciplinary case conferences and contribute to overall patient care coordination

Qualifications:

• Active and unrestricted Physical Therapist license in the state of practice
• Minimum of 1 year recent home health PT experience required; strong field background a plus
• Proficiency in Home Care Home Base (HCHB) and OASIS required
• Solid clinical judgment and efficient at managing a flexible schedule independently
• Valid drivers license, reliable vehicle, and auto insurance
• Exceptional assessment, communication, and organizational skills

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
