Provide expert physical therapy services in a home health setting, delivering personalized care to patients based on physician orders and thorough evaluations. This contract position offers a consistent Monday through Friday schedule from 8 am to 5 pm with no weekend or on-call duties, allowing a balanced work-life routine.

Responsibilities:
- Conduct comprehensive physical therapy evaluations using established tools and protocols.
- Develop and implement measurable treatment plans to enhance patient function and independence.
- Continuously assess patient progress, modifying treatment plans in collaboration with healthcare providers.
- Communicate effectively with medical teams, patients, and families about treatment goals and progress.
- Recommend and facilitate adaptive equipment to support rehabilitation efforts.
- Maintain timely and accurate documentation compliant with company and regulatory standards.
- Supervise Physical Therapist Assistants and Aides as necessary.
- Engage in ongoing education and participate in staff development initiatives.
Qualifications / Desired Experience:
- Licensure: Valid West Virginia PT license or compact license accepted.
- Experience preferred in home health and Home Care/Home Health Aide (HCHB) roles.
- Ability to meet the benchmark requirement of managing 28 units/pts per week.
- Skilled in adhering to HIPAA, corporate compliance, and professional practice standards.
Additional Details:
- Mileage reimbursement provided at $0.54 per mile.
- Coverage area includes Raleigh, Logan, and Wyoming counties.
