Job Description
Empower patients to regain independence and optimize their quality of life as a dedicated Home Health Physical Therapist. In this role, you will deliver comprehensive and compassionate physical therapy services directly to patients in their homes. You will play a vital role in helping individuals improve strength, mobility, and daily function, all while working independently and collaborating with a multidisciplinary healthcare team.
Schedule/Hours:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Approximately 2532 points per week (about 57 patient visits daily, based on visit types)
Key Responsibilities:
- Complete thorough physical therapy evaluations and OASIS assessments where applicable
- Develop personalized care plans focused on mobility, strength, balance, and safety
- Deliver therapeutic interventions such as gait, transfer training, fall prevention, and pain management
- Monitor and modify treatment plans to ensure optimal patient progress
- Educate patients and caregivers on home exercises, safety, and assistive device usage
- Maintain accurate, timely documentation in the Home Care Home Base (HCHB) system
- Work closely with nurses, occupational and speech therapists, and other team members for coordinated care
- Participate in interdisciplinary meetings and follow agency and regulatory standards
Qualifications:
- Active, in-good-standing Physical Therapist (PT) license in Oregon
- At least 1 year of recent home health physical therapy experience
- OASIS assessment experience required
- Proficiency with Home Care Home Base (HCHB) preferred
- Excellent clinical judgment, communication, and organizational abilities
- Reliable transportation, current drivers license, and auto insurance required
- Ability to adapt to a flexible, field-based work schedule
