Empower patients to regain independence and optimize their quality of life as a dedicated Home Health Physical Therapist. In this role, you will deliver comprehensive and compassionate physical therapy services directly to patients in their homes. You will play a vital role in helping individuals improve strength, mobility, and daily function, all while working independently and collaborating with a multidisciplinary healthcare team.

Schedule/Hours:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points per week (about 57 patient visits daily, based on visit types)

Key Responsibilities:

• Complete thorough physical therapy evaluations and OASIS assessments where applicable
• Develop personalized care plans focused on mobility, strength, balance, and safety
• Deliver therapeutic interventions such as gait, transfer training, fall prevention, and pain management
• Monitor and modify treatment plans to ensure optimal patient progress
• Educate patients and caregivers on home exercises, safety, and assistive device usage
• Maintain accurate, timely documentation in the Home Care Home Base (HCHB) system
• Work closely with nurses, occupational and speech therapists, and other team members for coordinated care
• Participate in interdisciplinary meetings and follow agency and regulatory standards

Qualifications:

• Active, in-good-standing Physical Therapist (PT) license in Oregon
• At least 1 year of recent home health physical therapy experience
• OASIS assessment experience required
• Proficiency with Home Care Home Base (HCHB) preferred
• Excellent clinical judgment, communication, and organizational abilities
• Reliable transportation, current drivers license, and auto insurance required
• Ability to adapt to a flexible, field-based work schedule
