Job Description

Deliver personalized, high-quality care as a skilled Home Health Physical Therapist, providing essential therapy services to patients in their homes. Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Qualifications:

  • Current Physical Therapist (PT) license in good standing
  • At least one year of recent home health physical therapy experience (field-based strongly preferred)
  • Proficiency with OASIS assessment and documentation requirements
  • Familiarity with Home Care Home Base (HCHB) electronic documentation system preferred
  • Strong clinical judgment, organizational, and time-management skills
  • Reliable personal transportation, valid drivers license, and auto insurance
  • Ability to work independently and manage a flexible, field-based workload
  • Excellent communication skills and collaborative spirit

Responsibilities:

  • Perform comprehensive in-home physical therapy evaluations, including required OASIS assessments
  • Design and implement individualized plans of care aimed at improving mobility, strength, balance, and functional independence
  • Deliver skilled interventions: gait, transfer, and balance training, fall prevention, and pain management
  • Regularly assess patient progress and adjust treatment plans as necessary in coordination with the healthcare team
  • Educate patients and caregivers in proper exercise execution, safety, and effective use of adaptive equipment
  • Accurately and promptly document all visits and care in HCHB
  • Communicate and coordinate with nurses, therapists, social workers, home health aides, and physicians to ensure effective, multidisciplinary case management
  • Participate in interdisciplinary meetings, case conferences, and ongoing professional development
