Make a significant impact in the lives of patients by providing high-quality physical therapy services in a home health setting. As a dedicated physical therapist, you will play a key role in helping individuals regain and maintain functional independence, working closely with each patient to develop and execute tailored treatment plans.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract assignment
• Approximately 2532 points per week (typically 57 patient visits per day)

Desired Experience and Qualifications:

• Current, active Physical Therapist (PT) license in the state
• At least 1 year of recent experience in home health physical therapy, with strong field-based skills
• Experience with OASIS documentation and assessments is required
• Home Care Home Base (HCHB) proficiency preferred
• Clinical expertise in gait training, neuromuscular re-education, and fall prevention
• Reliable transportation, valid drivers license, and auto insurance are essential
• Exceptional interpersonal, organizational, and communication skills
• Comfortable working independently in a field environment

Key Responsibilities:

• Perform thorough physical therapy evaluations and OASIS assessments within patient homes
• Develop and implement individualized plans of care aimed at improving strength, mobility, and safety
• Deliver a wide range of therapeutic interventions to maximize patient outcomes and prevent falls
• Monitor patient progress, adjusting the care plan and coordinating with physicians and healthcare team members as needed
• Provide patient and caregiver education on safety, assistive devices, and home exercise programs
• Maintain accurate, timely documentation in HCHB and participate in interdisciplinary case conferences
