Step into a meaningful healthcare role delivering physical therapy care to patients in their homes. In this contract position, youll use your clinical expertise to foster recovery and support patient independence, working alongside an interdisciplinary team to ensure comprehensive, coordinated care.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Qualifications:
- Active Physical Therapist (PT) license in good standing for the state of practice
- At least 1 year of recent experience in Home Health Physical Therapy (field experience preferred)
- Proficiency using Home Care Home Base (HCHB) is a plus
- OASIS assessment experience required
- Strong functional assessment and clinical decision-making skills
- Must possess reliable transportation, a valid drivers license, and auto insurance
- Independent work ethic with excellent communication and organizational skills
Primary Responsibilities:
- Conduct comprehensive physical therapy assessments, including OASIS evaluations as required
- Establish and implement patient-centered, individualized treatment plans focused on improving mobility, strength, pain management, balance, and facilitating safe home environments
- Deliver skilled interventions such as therapeutic exercise, transfer training, gait and neuromuscular re-education, and fall prevention education
- Regularly re-evaluate patient progress, updating plans and documenting all care in Home Care Home Base (HCHB)
- Educate patients and caregivers with an emphasis on safety, exercise, and use of assistive equipment
- Communicate and collaborate with peers across nursing, occupational therapy, speech therapy, and case management
- Attend case conferences, interdisciplinary meetings, and engage in continuing education as needed
- Maintain compliance with regulatory guidelines, Medicare, and agency policies
