Make a meaningful impact in the lives of patients by delivering high-quality care as a skilled Home Health Physical Therapist (PT). This contract opportunity offers a dynamic, field-based role serving patients in their own homes while supporting their journey to improved mobility, independence, and wellness.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract duration
- Typical caseload: 2532 points per week (about 57 home visits per day)
- Daily travel required within assigned territory (mileage reimbursement provided)
Key Qualifications:
- Active Physical Therapist (PT) license in good standing for the state
- Minimum of 1 year of recent home health PT experience (field-based strongly preferred)
- OASIS assessment experience required; proficiency with Home Care Home Base (HCHB) preferred
- Outstanding clinical assessment and problem-solving abilities
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to work independently with strong communication, organization, and time-management skills
What You'll Do:
- Complete comprehensive patient evaluations, including required OASIS assessments
- Develop and deliver individualized therapy plans of care focused on safe, effective functional restoration and mobility
- Provide skilled interventionstherapeutic exercise, gait/transfer training, neuromuscular re-education, fall prevention, and more
- Educate patients and caregivers on home exercise programs, safety, and adaptive equipment
- Accurately document visits and care in HCHB
- Collaborate within interdisciplinary care teams and participate in case conferences and continuing education
- Ensure compliance with all regulatory and Medicare home health standards
