Job Description
Deliver high-quality physical therapy in a home health setting, supporting patients in regaining mobility, independence, and confidence. Work with a diverse caseload and collaborate closely with other healthcare professionals to optimize care. This contract role offers the opportunity to make a real difference for patients while enjoying a field-based, flexible environment.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Approximately 2532 points per week (about 57 patient visits daily)
Responsibilities:
- Conduct comprehensive in-home physical therapy evaluations, including OASIS assessments when required.
- Develop and implement individualized plans of care focusing on functional mobility, strength, gait, pain management, and safety.
- Deliver therapeutic interventions: exercise, balance/fall prevention, transfer/gait training, and neuromuscular re-education.
- Monitor patient progress, adapt treatment plans as needed, and communicate clinical updates to the interdisciplinary care team.
- Educate patients and caregivers on exercise techniques, durable medical equipment, and safety strategies.
- Document visits and progress notes accurately and timely using Home Care Home Base (HCHB).
- Actively participate in case conferences and ongoing professional development.
Qualifications:
- Current, active Physical Therapist (PT) license in the state of practice (unrestricted).
- At least 1 year of recent home health physical therapy experience (fieldwork preferred).
- Proficiency with OASIS and Home Care Home Base (HCHB) EMR preferred.
- Reliable transportation, valid drivers license, and adequate auto insurance.
- Demonstrated ability to work independently with strong time management and organizational skills.
- Excellent clinical judgment and communication abilities.
