Deliver high-quality patient care by joining a dedicated healthcare team as a Home Health Physical Therapist. In this contract role, you will provide essential physical therapy services directly in patients homes, focusing on enhancing their mobility, independence, and quality of life. This opportunity allows you to work autonomously while collaborating closely with a multidisciplinary group of professionals in a supportive environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Daily travel required within the assigned territory (mileage reimbursement provided)
• Caseload: Approximately 2532 points/week (57 patient visits per day)

Qualifications:

• Current, active Physical Therapist (PT) license in good standing
• At least one year of recent home health experience preferred
• Strong assessment skills, with required OASIS experience
• Proficiency in Home Care Home Base (HCHB) electronic documentation strongly preferred
• Solid clinical judgment, organizational, and time management skills
• Reliable transportation, valid drivers license, and active auto insurance
• Ability to work independently and adapt to changing schedules

Key Responsibilities:

• Perform comprehensive in-home physical therapy evaluations, including OASIS assessments
• Develop and execute individualized treatment plans tailored to patient goals
• Employ a range of interventions: therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention
• Monitor and document progress, modifying plans as necessary
• Communicate effectively with care team, patients, and caregivers
• Maintain compliance with all policy, regulatory, and Medicare guidelines
• Educate patients and families on safe mobility, home exercises, and use of assistive devices

$2,414 - $2,554/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
