Job Description

An experienced Home Health Physical Therapist (PT) is needed to provide skilled therapy services in patients homes. This role focuses on improving patients mobility, strength, balance, and independence through personalized care plans. Candidates should be self-directed, organized, and capable of delivering compassionate, high-quality physical therapy services in an independent, field-based setting.

Responsibilities:

  • Conduct comprehensive PT evaluations, including OASIS assessments when applicable.
  • Develop and implement individualized treatment plans targeting mobility, strength, gait, pain management, and safety.
  • Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
  • Monitor patient progress, adjust plans as needed, and communicate with physicians and care teams.
  • Educate patients and caregivers on exercises, safety techniques, assistive device usage, and home exercise regimens.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB).
  • Collaborate with nurses, OT, SLP, medical social workers, and home health aides to coordinate care.
  • Participate in interdisciplinary meetings and adhere to agency policies and Medicare guidelines.

Qualifications:

  • Active Physical Therapist license in good standing for the practicing state.
  • Minimum 1 year of recent home health PT experience; field experience preferred.
  • Proficiency with Home Care Home Base (HCHB) preferred.
  • OASIS assessment experience required.
  • Strong clinical decision-making and functional assessment skills.
  • Valid drivers license, reliable transportation, and auto insurance.
  • Ability to work independently and manage a flexible schedule.
  • Excellent communication, organization, and time management skills.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM
