A skilled Home Health Physical Therapist (PT) is needed to provide specialized physical therapy care directly in patient homes in the Avondale, CO area. This contract role offers a dynamic opportunity to improve patient mobility, strength, balance, and overall independence through personalized, hands-on therapeutic interventions.

Responsibilities include conducting comprehensive evaluations using OASIS assessments, developing and implementing customized treatment plans focused on functional outcomes, and delivering skilled interventions such as therapeutic exercise, gait and transfer training, and fall prevention. You will educate patients and caregivers, monitor progress toward goals, adjust treatment as needed, and collaborate closely with an interdisciplinary team to ensure coordinated care. Accurate and timely documentation using Home Care Home Base (HCHB) is essential. Adherence to agency policies and Medicare home health standards is required.

Key Qualifications:

• Active Physical Therapist license in the state of practice in good standing
• Minimum 1 year of recent home health PT experience, with field experience preferred
• Proficiency with Home Care Home Base (HCHB) preferred
• Experience conducting OASIS assessments is required
• Strong clinical decision-making and functional assessment capabilities
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently with a flexible field-based schedule
• Excellent communication, organizational, and time-management skills

Schedule & Location:

• Full-time contract, 13 weeks
• Monday through Friday, 8:00 AM to 5:00 PM
• Field-based work visiting patient homes across the assigned territory in Avondale, Colorado
• Mileage reimbursement provided
