An experienced and compassionate Physical Therapist is needed to deliver skilled home health physical therapy services to patients in their residences within the Atlanta area. This role centers on improving patient mobility, strength, balance, and overall functional independence through personalized care plans. The ideal candidate thrives in an independent, field-based setting and is committed to high-quality, patient-focused therapy.

Responsibilities:
- Conduct thorough physical therapy evaluations, including required OASIS assessments.
- Develop and execute individualized treatment plans emphasizing mobility, gait, strength, pain management, and safety.
- Administer skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention techniques.
- Monitor patient progress and modify plans when necessary while maintaining open communication with physicians and team members.
- Educate patients and caregivers on exercise regimens, safety protocols, assistive device use, and home exercise programs.
- Ensure prompt and accurate documentation using the Home Care Home Base (HCHB) system.
- Collaborate with interdisciplinary teams, including nursing and social work professionals.
- Participate in case conferences and adhere strictly to regulatory guidelines and agency policies.
Qualifications & Desired Experience:
- Active Physical Therapist license in the state of practice in good standing.
- Minimum of one year recent home health physical therapy experience, preferably in a field-based role.
- Proficiency with Home Care Home Base (HCHB) and OASIS documentation tools.
- Strong clinical reasoning and functional assessment capabilities.
- Reliable transportation with a valid drivers license and insurance.
- Self-motivated, organized, excellent communication and time management skills.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Approximate caseload: 57 patient visits per day (2532 weekly points)
