Job Description

A skilled Physical Therapist with a passion for delivering personalized care in patients homes is needed for a full-time, 13-week contract opportunity in Aiken, SC. This role involves collaborating closely with physicians, nurses, and occupational therapists to develop and implement treatment plans that enhance patient mobility and independence.

Responsibilities:

  • Perform comprehensive assessments of patients physical conditions, functional abilities, and mobility challenges.
  • Develop and execute individualized treatment plans aligned with patient goals and physician directives.
  • Provide therapeutic exercises, manual therapy, and modalities to improve strength and range of motion.
  • Educate patients and families on exercise techniques, transfers, and use of assistive devices.
  • Continuously monitor and document patient progress, adjusting plans as needed.
  • Communicate regularly with healthcare team members to ensure coordinated and effective care.
  • Maintain accurate electronic medical records following state and federal regulations.
  • Comply with professional, ethical, and regulatory standards.

Qualifications:

  • Bachelors or Masters degree in Physical Therapy from an accredited institution.
  • Current state license or eligibility to practice as a Physical Therapist.
  • Experience in a home health setting.
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical records.
  • Strong clinical reasoning and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Knowledge of Medicare regulations related to home health care.
  • Valid drivers license and reliable transportation to visit patients.
