Job Description

Experienced and compassionate professionals are needed to provide high-quality home health physical therapy services to patients aiming to improve their mobility, strength, and functional independence. This rewarding contract role emphasizes autonomy and patient-centered care in a dynamic field-based setting.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct comprehensive physical therapy evaluations, including OASIS assessments as indicated by patient needs.
  • Develop and implement individualized care plans focused on mobility, balance, strength, safety, pain management, and fall prevention.
  • Deliver skilled interventions including therapeutic exercise, neuromuscular re-education, gait and transfer training.
  • Evaluate and document patient progress, revising treatment strategies and communicating updates to physicians and the healthcare team.
  • Educate patients and their caregivers on home exercise programs, safety strategies, and use of assistive devices.
  • Maintain accurate, timely documentation within the Home Care Home Base (HCHB) system.
  • Collaborate with nurses, therapists, medical social workers, and home health aides to support coordinated care.
  • Participate in case conferences, interdisciplinary meetings, and ongoing education as needed.
  • Uphold all professional, regulatory, and agency standards throughout the provision of care.

Qualifications:

  • Active, unrestricted Physical Therapist (PT) license in the state.
  • At least 1 year of recent home health physical therapy experience, with field experience strongly preferred.
  • OASIS assessment experience required.
  • Home Care Home Base (HCHB) proficiency preferred.
  • Excellent clinical judgment, assessment, and organizational skills.
  • Strong verbal and written communication abilities.
  • Reliable transportation, a valid drivers license, and current auto insurance.
  • Ability to work independently with a flexible approach to patient scheduling.
