Job Description

Deliver skilled physical therapy services directly in patients homes as a Home Health Physical Therapist (PT), focusing on enhancing mobility, strength, balance, and functional independence. This full-time, 13-week contract role based in Adams, MA offers an opportunity to work autonomously while collaborating with a dedicated interdisciplinary team to support positive patient outcomes.

Responsibilities:

  • Perform comprehensive physical therapy evaluations, including OASIS assessments (Start of Care, Resumption, Recertification, Discharge).
  • Develop and implement personalized treatment plans targeting functional mobility, strength, gait, pain management, and safety.
  • Provide skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention techniques.
  • Monitor patient progress and adjust care plans as necessary while communicating updates to physicians and care teams.
  • Educate patients and caregivers on exercises, safety protocols, assistive device use, and home exercise programs.
  • Maintain accurate and timely documentation using Home Care Home Base (HCHB).
  • Collaborate with nurses, occupational therapists, speech pathologists, medical social workers, and home health aides to ensure coordinated care.
  • Participate in case conferences, interdisciplinary meetings, and continuing education.
  • Comply with agency policies, regulatory standards, and Medicare home health guidelines.

Qualifications:

  • Active Physical Therapist license in good standing for the state of practice.
  • Minimum of 1 year recent home health physical therapy experience; field experience preferred.
  • OASIS assessment experience required.
  • Proficiency with Home Care Home Base (HCHB) preferred.
  • Strong clinical decision-making and functional assessment skills.
  • Reliable transportation with valid drivers license and auto insurance.
  • Ability to work independently and manage a flexible, field-based schedule.
  • Excellent communication, time management, and organizational skills.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM
  • Caseload approximately 2532 points weekly (57 patient visits/day)
  • Daily travel within assigned territory (mileage reimbursement provided)
