Job Description
Step into a rewarding role providing critical physical therapy care in a home health setting. This contract position focuses on delivering high-quality, patient-centered therapy services to individuals in the comfort of their homes, empowering them to reach greater levels of mobility, independence, and safety.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- 13-week contract duration
- Daily travel to patients' residences within an assigned territory
Key Qualifications:
- Active, good-standing Physical Therapist (PT) license in the state
- At least one year of recent home health physical therapy experience (field experience preferred)
- Proficient with Home Care Home Base (HCHB) electronic documentation (preferred)
- OASIS assessment experience required
- Strong assessment, clinical decision-making, and functional mobility skills
- Able to work independently with effective time management
- Valid drivers license, reliable transportation, and auto insurance
Primary Responsibilities:
- Conduct comprehensive physical therapy evaluationsincluding OASIS assessments as needed
- Develop and implement customized care plans to address strength, balance, gait, and safety
- Provide skilled interventions such as therapeutic exercise, neuromuscular re-education, transfer training, and fall prevention
- Monitor patient progress and adjust treatment plans as appropriate
- Educate patients and caregivers regarding safety, exercises, and assistive device use
- Maintain punctual, accurate documentation in HCHB system
- Collaborate with interdisciplinary team members to ensure coordinated, effective care
- Participate in meetings, case conferences, and required professional development
- Uphold all agency, regulatory, and Medicare home health guidelines
