Job Description

A skilled and compassionate Occupational Therapist (OT) is needed to deliver essential home health services, supporting patients in their own environment. In this full-time, contract role, you will empower patients to achieve greater independence with daily activities and mobility, collaborating as part of an interdisciplinary team that prioritizes high-quality, patient-centered care.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM

Responsibilities:

  • Perform comprehensive home-based OT evaluations, including OASIS assessments when indicated.
  • Develop and implement care plans focused on ADLs, IADLs, upper extremity rehabilitation, cognitive function, and home safety.
  • Deliver skilled interventions: therapeutic activities, self-care training, adaptive equipment use, energy conservation strategies, and environmental modifications.
  • Recommend appropriate adaptive or durable medical equipment (DME) based on patient needs.
  • Assess patient progress, update care plans as needed, and communicate with physicians and interdisciplinary colleagues.
  • Educate patients and caregivers about safety awareness, fall prevention, and effective compensatory techniques.
  • Complete documentation accurately and promptly using the Home Care Home Base (HCHB) electronic system.
  • Participate in case conferences and interdisciplinary meetings, contributing to coordinated patient care.
  • Ensure compliance with agency policies, Medicare, and regulatory guidelines.

Qualifications:

  • Active Occupational Therapist (OT) license in good standing in the state of practice.
  • At least 1 year of recent, relevant home health OT experience (field experience strongly preferred).
  • Proficiency in Home Care Home Base (HCHB) preferred; OASIS assessment experience required.
  • Excellent assessment and functional training skills.
  • Strong organization, communication, and time management abilities.
  • Reliable transportation, current drivers license, and auto insurance.
  • Independence and flexibility in a field-based setting.
