Youll gain meaningful exposure to a range of patient profilesexclusively adult and geriatricacross Racine and Kenosha Counties, supporting individuals transitioning through post-stroke recovery, joint replacement, deconditioning, COPD, CHF, and more. Working hand-in-hand with experienced RN and PT teams, daily collaboration fosters ongoing skill development, while adopting best practices from multiple professionals diversifies your resume and prepares you for future leadership.

Schedule/Hours

  • Monday Friday, approximately 7:45 AM 4:00 PM
  • No weekends, on-call, or holiday shifts
  • Flexible RTO approval through management

Qualifications

  • Active WI Occupational Therapy license
  • Current BLS certification
  • Minimum 3 years' OT experience (SNF, home health, or acute care; home health preferred)
  • Valid drivers license, reliable vehicle, and active auto insurance
  • Bachelors degree in Occupational Therapy accepted
  • General OT evaluation and treatment expertise, with a focus on ADLs and adaptive equipment
  • EPIC EMR experience preferred (training provided)

Key Responsibilities

  • Complete six patient visits/points daily; evaluations are weighted at 1.5
  • Deliver quality home health OT care for adults in Racine and Kenosha Counties
  • Participate in daily interdisciplinary team huddles via phone
  • Coordinate care with RN and PT leaders, supervisors, and peers
  • Navigate varying distances (majority within 20-30 minutes drive)
