Job Description

Deliver exceptional patient care as a Home Health Occupational Therapist in a field-based setting. Provide services to individuals in their homes, prioritizing independence, functional improvement, and safety for patients recovering from illness, injury, or surgery. Utilize clinical expertise to evaluate needs, develop targeted therapy plans, and guide patients through their rehabilitation journey.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • 13-week contract assignment

Key Responsibilities:

  • Conduct comprehensive assessments, including OASIS documentation for start of care, recertification, resumption, and discharge when applicable.
  • Design and implement individualized plans of care emphasizing ADLs, instrumental ADLs, upper extremity function, cognition, and home modifications.
  • Deliver skilled therapy interventions such as therapeutic activities, training in self-care, use of adaptive equipment, and energy conservation.
  • Evaluate, recommend, and educate on durable medical equipment and assistive devices.
  • Document patient care accurately and timely in Home Care Home Base (HCHB).
  • Collaborate with an interdisciplinary care team, participating in meetings and case conferences to drive high-quality, coordinated care.
  • Provide ongoing education and resources to patients and their caregivers on fall prevention, functional mobility, and home safety.
  • Remain compliant with agency policies, regulatory standards, and Medicare guidelines.

Desired Skills & Qualifications:

  • Valid Occupational Therapist (OT) license in state of practice
  • At least 1 year of recent home health OT experience (field-based strongly preferred)
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools
  • Strong assessment, clinical reasoning, and patient education skills
  • Highly organized with the ability to work independently
  • Reliable vehicle, valid drivers license, and auto insurance required

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
