A home health Occupational Therapist (OT) role is available to deliver expert patient-centered care in a field-based setting. This position focuses on improving patients' abilities to perform activities of daily living (ADLs), enhancing safety and independence within their home environments.
Key Responsibilities:
- Conduct comprehensive occupational therapy evaluations, including OASIS assessments when applicable.
- Develop and implement individualized care plans addressing ADLs, instrumental ADLs (IADLs), cognition, upper extremity function, and home safety.
- Provide skilled therapeutic interventions such as self-care training, energy conservation techniques, adaptive equipment instruction, and environmental modifications.
- Assess and recommend appropriate durable medical equipment (DME) tailored to patient needs.
- Monitor patient progress, adjust treatments accordingly, and communicate updates to physicians and the interdisciplinary team.
- Educate patients and caregivers on fall prevention, safety awareness, and compensatory strategies.
- Maintain accurate and timely documentation using Home Care Home Base (HCHB) software.
- Collaborate with nurses, physical therapists, speech-language pathologists, social workers, and home health aides for coordinated care.
- Participate in case conferences, professional development, and ensure compliance with agency policies and Medicare guidelines.
Qualifications:
- Active Occupational Therapist license in good standing within the state of practice.
- Minimum of 1 year recent home health OT experience; field-based experience preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments required.
- Strong clinical assessment, functional training, and patient education skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently with excellent organizational and communication skills.
Schedule & Contract Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 2532 points weekly (57 patient visits per day)
- Field-based with daily travel to patient homes within assigned territory; mileage reimbursement provided.
