Job Description

Deliver essential care in the community as a Home Health Occupational Therapist, providing direct therapy services in patients homes. This contract opportunity focuses on helping individuals regain independence, safety, and improved function through meaningful occupational therapy interventions in a home health setting.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct thorough occupational therapy assessments, including completion of OASIS documentation for admissions, follow-up, and discharge.
  • Create and implement individualized treatment plans focusing on activities of daily living (ADLs), upper extremity function, cognition, mobility, and safety.
  • Deliver skilled interventions such as self-care training, energy conservation, adaptive equipment education, and environmental modifications.
  • Assess the need for and recommend appropriate durable medical equipment.
  • Educate patients and caregivers on adaptive strategies, home safety, and compensatory techniques for optimum independence.
  • Document clinical findings accurately and promptly in the Home Care Home Base (HCHB) platform.
  • Collaborate closely with an interdisciplinary healthcare team, participating in case conferences and professional meetings.
  • Maintain compliance with internal policies, regulatory standards, and Medicare home health guidelines.
  • Travel locally to patient homes each day (mileage reimbursement available).

Desired Qualifications:

  • Active Occupational Therapist license (OT) in good standing.
  • At least 1 year of recent home health OT experience; strong field-based clinical skills highly valued.
  • Experience with OASIS documentation and proficiency in the Home Care Home Base (HCHB) system preferred.
  • Reliable transportation, valid drivers license, and current auto insurance.
  • Organized, excellent communicator, and capable of working independently.
