Job Description

Deliver high-impact occupational therapy care directly to patients in a home setting as a dedicated Home Health Occupational Therapist. Collaborate with an interdisciplinary team while supporting patients independence, safety, and daily functioning. This contract-based role allows you to truly make a difference, helping individuals reach their rehabilitation goals in a familiar environment.

Schedule:

  • Monday through Friday: 8:00 AM 5:00 PM

Key Responsibilities:

  • Complete comprehensive occupational therapy evaluations, including OASIS assessments when needed.
  • Design and implement personalized care plans focused on activities of daily living (ADLs), instrumental ADLs, upper extremity function, cognition, and home safety.
  • Provide evidence-based interventions, such as therapeutic exercises, energy conservation techniques, and adaptive equipment training.
  • Recommend and assist with acquisition of appropriate durable medical equipment (DME).
  • Monitor patient progress and update care plans, collaborating with physicians and the care team for optimal outcomes.
  • Educate patients and caregivers on functional independence, safety precautions, and compensatory strategies.
  • Document all patient care activities thoroughly and efficiently in the Home Care Home Base (HCHB) electronic system.
  • Attend case conferences, interdisciplinary meetings, and engage in ongoing professional development to maintain best practices.
  • Maintain regulatory compliance and uphold agency policies and Medicare home health standards.

Qualifications:

  • Current Occupational Therapist license (in good standing) for the state of practice.
  • At least 1 year of recent experience in home health occupational therapy (field experience preferred).
  • Required experience with OASIS assessments; proficiency in HCHB is preferred.
  • Exceptional assessment, time management, and communication skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to excel independently within a flexible, field-based schedule.
