Make a meaningful impact on patients lives by delivering exceptional occupational therapy services directly in their homes. As a Home Health Occupational Therapist on a full-time, 13-week contract, you will provide expert care and empower individuals to maximize independence and safety in their daily routines.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Role Overview:
- Conduct detailed occupational therapy evaluations and OASIS assessments as required.
- Develop and implement individualized care plans addressing ADLs, IADLs, cognitive function, upper extremity function, and home safety.
- Provide therapeutic interventions, adaptive equipment training, environmental modifications, and self-care education.
- Recommend and coordinate appropriate use of durable medical equipment (DME).
- Monitor progress, update care plans, and communicate with physicians and interdisciplinary team members.
- Educate patients and caregivers on safe mobility, fall prevention, and compensatory strategies to enhance functional independence.
- Complete accurate and timely clinical documentation in Home Care Home Base (HCHB).
- Attend case conferences and interdisciplinary meetings; maintain regulatory compliance.
- Daily travel within the assigned territory (mileage reimbursement provided).
Preferred Qualifications:
- Active Occupational Therapist license in good standing.
- At least one year of recent Home Health OT experience.
- Proficiency with Home Care Home Base (HCHB) and required OASIS assessment skills.
- Strong clinical assessment, organizational, and communication abilities.
- Reliable transportation, valid drivers license, and auto insurance.
- Demonstrated ability to manage a flexible, field-based schedule and work autonomously.

Compensation for this position ranges from $25.97 to $50.00. Eligible candidates may also receive healthcare benefits, housing, and meals/incidentals. Full details of our healthcare benefits plan can be found here. This posting is open for 60 days after the posting date.
