Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM

What youll do:

• Perform comprehensive occupational therapy assessments, including OASIS when required.
• Develop evidence-based treatment plans focused on activities of daily living, instrumental tasks, upper extremity function, cognition, and home safety.
• Deliver skilled interventions such as adaptive equipment training, therapeutic activities, self-care retraining, and environmental modifications.
• Recommend durable medical equipment and adaptive tools to maximize patient independence.
• Adjust care plans as needed based on ongoing assessment and patient progress.
• Educate patients and caregivers in safe mobility, fall prevention, and effective compensatory strategies.
• Document all visits and patient outcomes in Home Care Home Base (HCHB).
• Collaborate with an interdisciplinary team, participating in case conferences and care plan discussions.
• Ensure adherence to regulatory standards and Medicare home health guidelines.
• Travel daily within your assigned territory with provided mileage reimbursement.

Qualifications:

• Valid, unrestricted Occupational Therapist license in state of practice.
• At least 1 year of recent Home Health OT experience.
• Familiarity with OASIS and proficiency in HCHB preferred.
• Sound clinical reasoning and outstanding functional training abilities.
• Strong organization, communication, and independent work skills.
• Reliable transportation, a valid drivers license, and current auto insurance.

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
