Bring your passion for patient-centered care to a dynamic Home Health role, providing vital occupational therapy services in the comfort of patients homes. As an Occupational Therapist in this contract position, youll enjoy a regular weekday scheduleMonday to Friday, 8:00 AM to 5:00 PMserving a caseload of approximately 2532 points per week (typically 57 visits daily).
What Youll Do:
- Conduct comprehensive OT evaluations, including all required OASIS assessments (Start of Care, Resumption, Recertification, Discharge)
- Develop and oversee tailored treatment plans, focusing on ADLs, IADLs, upper extremity strength and function, cognitive skills, and home safety
- Deliver targeted interventions: therapeutic activities, self-care training, energy conservation, equipment and adaptive device use, and home modifications
- Recommend and assess the need for durable medical equipment (DME)
- Adjust care plans based on patient progress and ongoing assessment findings
- Educate patients and caregivers on functional independence, safety strategies, fall prevention, and home adaptations
- Maintain clear, timely documentation using Home Care Home Base (HCHB)
- Coordinate closely with the care team, including nurses, PTs, SLPs, social workers, and aides
- Attend case conferences and contribute to interdisciplinary meetings and professional growth activities
- Ensure compliance with all agency, regulatory, and Medicare guidelines
What Were Looking For:
- Active Occupational Therapist (OT) license in good standing
- At least 1 year of recent Home Health OT experience
- Strong familiarity with OASIS and HCHB documentation systems preferred
- Strong clinical, assessment, and communication skills
- Reliable transportation, valid drivers license, and proof of auto insurance
- Ability to thrive independently and manage a variable, field-based schedule
