Job Description
Deliver high-impact care as a Home Health Occupational Therapist, providing expert therapy services directly in patients homes. This contract opportunity offers meaningful, one-on-one patient interactions and the chance to help individuals regain independence and enhance their quality of life through purposeful interventions and education.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Responsibilities:
- Conduct thorough occupational therapy evaluations in patients homes, including OASIS assessments as required.
- Develop and tailor individualized care plans that promote improved ADLs, IADLs, upper extremity function, home safety, and cognitive skills.
- Deliver targeted interventions such as self-care training, therapeutic activities, energy conservation education, and environmental modifications.
- Recommend and oversee use of adaptive equipment and durable medical equipment (DME) based on ongoing assessment.
- Monitor progress, update treatment plans as needed, and maintain clear communication with physicians and interdisciplinary colleagues.
- Educate patients and caregivers on adaptive strategies, safety measures, functional independence, and fall prevention.
- Accurately and promptly document all visits and interventions using Home Care Home Base (HCHB).
- Participate in case conferences, team meetings, and professional development activities.
- Uphold compliance with agency policies, Medicare guidelines, and industry regulations.
Qualifications:
- Active and unencumbered Occupational Therapist (OT) license for the state of practice
- At least 1 year of recent Home Health experience as an Occupational Therapist (field-based strongly preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments
- Reliable transportation, valid driver's license, and insurance
- Excellent time management, organizational, and communication skills
- Ability to independently manage a flexible, field-based schedule
