Job Description

Seeking an experienced and compassionate professional with expertise in home health occupational therapy. This position involves delivering high-quality therapy to patients in their homes, concentrating on enhancing independence, safety, and daily living skills. The ideal candidate excels at working autonomously, demonstrates strong clinical judgement, and enjoys contributing to a collaborative, interdisciplinary care team.

Schedule & Hours:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract

Key Responsibilities:

  • Perform comprehensive occupational therapy evaluations, including OASIS assessments as needed.
  • Develop and execute patient-centered plans of care addressing activities of daily living, cognition, upper extremity function, and safety.
  • Provide skilled therapeutic interventions such as energy conservation, adaptive equipment training, and environmental modifications.
  • Assess and recommend appropriate durable medical equipment to support patient needs.
  • Monitor progress and adapt care plans, communicating updates with physicians and care team members.
  • Educate patients and caregivers on home safety, compensatory strategies, and fall prevention.
  • Document all care accurately and efficiently in Home Care Home Base (HCHB).
  • Collaborate with nurses, physical therapists, speech-language pathologists, social workers, and aides for coordinated care.
  • Participate in case conferences, team meetings, and ongoing professional development.
  • Ensure compliance with regulatory and agency guidelines.

Qualifications:

  • Active Occupational Therapist (OT) license in good standing.
  • Minimum 1 year of recent home health OT experience; field-based experience preferred.
  • Working knowledge of Home Care Home Base (HCHB) and OASIS required.
  • Strong assessment, organizational, and communication skills.
  • Reliable transportation, valid drivers license, and auto insurance.
