Join a dynamic healthcare team as a Home Health Occupational Therapist focused on delivering compassionate, high-quality care in patient homes. This full-time contract role offers an opportunity to make a significant impact by enhancing patient independence and safety through expert occupational therapy services.

Key Responsibilities:

• Perform comprehensive occupational therapy evaluations and complete OASIS assessments for Start of Care, Resumption, Recertification, and Discharge as applicable.
• Develop and implement individualized treatment plans targeting activities of daily living (ADLs), instrumental ADLs (IADLs), upper extremity function, cognition, and home safety.
• Deliver skilled interventions including therapeutic activities, self-care training, energy conservation, adaptive equipment use, and environmental modifications.
• Assess patients needs for adaptive and durable medical equipment and recommend appropriate resources.
• Monitor patient progress, update plans accordingly, and maintain communication with physicians and interdisciplinary team members.
• Educate patients and caregivers on functional independence, fall prevention, safety techniques, and compensatory strategies.
• Document accurately and timely using the Home Care Home Base (HCHB) electronic system.
• Collaborate with nurses, physical therapists, speech-language pathologists, social workers, and home health aides to coordinate comprehensive care.
• Ensure compliance with regulatory standards and Medicare home health guidelines.

Qualifications:

• Current, active Occupational Therapist license in good standing.
• Minimum one year recent experience in Home Health Occupational Therapy; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
• Strong clinical assessment, communication, time management, and organizational skills.
• Reliable transportation, valid drivers license, and insurance to support travel within assigned territory.
• Ability to work independently and manage a flexible field-based schedule.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
