Deliver essential occupational therapy services directly to patients in a home health setting, focusing on promoting independence and improving quality of life. This field-based opportunity is ideal for a highly skilled and organized Occupational Therapist dedicated to delivering individualized care. The role requires collaboration with an interdisciplinary team and emphasizes patient and caregiver education on adaptive strategies and home safety.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active Occupational Therapist (OT) license in good standing for the state of practice
- At least 1 year of recent Home Health OT experience (field-based experience highly valued)
- Proficient with Home Care Home Base (HCHB) electronic documentation preferred
- Strong knowledge and experience completing OASIS assessments
- Excellent clinical, assessment, and patient training abilities
- Reliable transportation, current drivers license, and valid auto insurance
- Comfortable working independently and managing a flexible caseload
- Exceptional communication, time management, and organizational skills
Primary Responsibilities:
- Conduct comprehensive OT evaluations, including required OASIS assessments
- Develop and implement individualized treatment plans targeting ADLs, IADLs, cognitive skills, and home safety
- Deliver skilled OT interventions (e.g., therapeutic activities, self-care, energy conservation, adaptive equipment training)
- Recommend and assess needs for durable medical equipment (DME)
- Track and report patient progress, adapting care plans as necessary
- Educate patients and caregivers in safe, effective adaptive strategies and use of assistive equipment
- Maintain thorough, timely documentation using HCHB
- Participate in case conferences and collaborative interdisciplinary meetings
- Ensure compliance with regulatory standards and agency procedures
