Job Description

Take on a rewarding role providing vital in-home therapy to patients as a Home Health Occupational Therapist. This contract engagement focuses on empowering clients to maximize their independence, safety, and quality of life in the comfort of their own homes. Working within a supportive interdisciplinary team, youll play a crucial part in helping patients achieve their goals through personalized, evidence-based interventions.

Schedule & Details:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Daily travel to patient homes within an assigned territory (mileage reimbursement provided)
  • Approximately 2532 points per week; typically 57 patient visits per day

Core Responsibilities:

  • Perform comprehensive occupational therapy evaluations, including OASIS assessments for various stages of care
  • Develop and execute individualized treatment plans targeting ADLs, IADLs, cognition, safety, and upper extremity function
  • Implement skilled interventions such as therapeutic activities, self-care and energy conservation training, adaptive equipment, and environmental modifications
  • Monitor progress, make clinical recommendations, and communicate updates across the care team
  • Educate patients and caregivers on adaptive techniques, fall prevention, and optimal use of assistive devices
  • Complete and maintain timely electronic documentation using Home Care Home Base (HCHB)
  • Ensure best-practice standards and compliance with regulatory guidelines

Qualifications:

  • Active, good standing Occupational Therapist (OT) license in state of practice
  • Minimum one year of recent field-based home health OT experience
  • Proficient with Home Care Home Base (HCHB) documentation (preferred)
  • Required OASIS assessment experience
  • Able to work independently and manage a dynamic schedule
  • Valid drivers license, reliable transportation, and auto insurance
  • Strong assessment, organizational, and communication skills
