Job Description

Deliver essential care as a Home Health Occupational Therapist, providing crucial therapy services directly in patients homes. This contract position plays a key role in supporting patients independence, safety, and ability to manage daily activities. Youll perform in-depth assessments, develop care plans, and collaborate with an interdisciplinary team to maximize patient outcomes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • 13-week contract duration
  • Caseload of approximately 2532 points weekly (typically 57 visits per day)

Qualifications:

  • Current Occupational Therapist license in good standing for the state
  • At least 1 year of recent field-based Home Health Occupational Therapy experience
  • Proficiency with Home Care Home Base (HCHB) preferred
  • Must be skilled in OASIS assessment and documentation
  • Sound clinical judgment and functional training expertise
  • Reliable vehicle, valid drivers license, and active auto insurance
  • Capable of working autonomously and managing schedule in the field
  • Clear communication and strong organizational abilities

Responsibilities:

  • Conduct comprehensive OT evaluations, including required OASIS assessments
  • Create and implement individualized care plans based on functional needs
  • Deliver interventions such as therapeutic activities, self-care training, adaptive equipment education, and home environment modifications
  • Recommend and arrange for appropriate durable medical equipment (DME)
  • Track patient progress, adapt care plans, and communicate updates with the clinical team
  • Educate patients and caregivers about functional independence and safe practices
  • Complete accurate, timely documentation in HCHB
  • Work in tandem with nurses, therapists, and other team members to coordinate care
  • Participate in case conferences, meetings, and professional development
  • Ensure all care complies with agency and Medicare standards
