Job Description

A skilled and compassionate Occupational Therapist (OT) is sought to provide home health services focusing on improving patients' ability to perform activities of daily living (ADLs) and enhancing independence and safety. This full-time, 13-week contract role is based in Rolla, MO, offering a professional challenge in a field-based setting with patient visits in their homes.

Key Responsibilities:

  • Conduct comprehensive occupational therapy evaluations, including OASIS assessments.
  • Develop and implement individualized care plans targeting ADLs, IADLs, cognition, upper extremity function, and home safety.
  • Provide skilled interventions such as therapeutic activities, adaptive equipment training, energy conservation, and environmental modifications.
  • Assess and recommend durable medical equipment (DME) as needed.
  • Monitor patient progress, update treatment plans, and maintain communication with physicians and interdisciplinary team members.
  • Educate patients and caregivers on fall prevention, safety awareness, and compensatory techniques.
  • Complete accurate documentation using Home Care Home Base (HCHB).
  • Collaborate closely with nurses, physical therapists, speech-language pathologists, and social workers to coordinate care.
  • Ensure compliance with regulatory standards and Medicare guidelines.

Qualifications/Desired Experience:

  • Active OT license in good standing within the state.
  • Minimum of 1 year recent home health OT experience, with field experience preferred.
  • Proficiency with OASIS assessments is required; experience with HCHB preferred.
  • Reliable transportation, valid drivers license, and auto insurance mandatory.
  • Strong clinical assessment skills and ability to manage a flexible, independent schedule.
  • Excellent communication, organizational, and time management skills.

Schedule/Hours:

  • Monday Friday, 8:00 AM 5:00 PM
  • Caseload: Approximately 2532 points weekly (57 patient visits per day).
