Job Description

Elevate patient care and foster independence as a Home Health Occupational Therapist by providing essential therapy services directly in patients homes. This contract opportunity is ideal for an organized, driven clinician who values autonomy and enjoys making a tangible difference in patients daily lives.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM

What youll do:

  • Conduct comprehensive occupational therapy assessments, including OASIS for start of care, recertifications, resumptions, and discharges.
  • Develop and implement individualized care plans with emphasis on ADLs, IADLs, cognition, upper extremity rehabilitation, and home safety.
  • Deliver skilled interventions such as self-care training, therapeutic activities, and adaptive equipment education.
  • Assess the need for adaptive equipment, recommend appropriate DME, and guide environmental modifications to promote safety.
  • Track patient progress, revise care plans as needed, and communicate closely with physicians and interdisciplinary team members.
  • Educate patients and families on fall prevention, safety, and compensatory strategies to maximize functional independence.
  • Complete accurate, timely documentation using Home Care Home Base (HCHB).
  • Effectively collaborate with other home health professionals for coordinated patient care.
  • Participate in case conferences, professional development, and interdisciplinary meetings as required.
  • Ensure full compliance with Medicare regulations, agency standards, and best practice guidelines.

Qualifications:

  • Active OT license in good standing.
  • At least 1 year of recent Home Health Occupational Therapy field experience.
  • OASIS assessment experience required; HCHB proficiency preferred.
  • Strong clinical assessment, communication, and time management skills.
  • Self-motivated with reliable transportation, current drivers license, and auto insurance.
