Job Description

A skilled and compassionate Home Health Occupational Therapist is needed to provide high-quality occupational therapy services to individuals in their homes. This role is perfect for self-motivated professionals who excel working independently and have a passion for helping patients regain independence in their daily lives.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct comprehensive occupational therapy evaluations using OASIS, when applicable.
  • Develop and implement individualized care plans addressing ADLs, IADLs, upper extremity function, cognition, and home safety.
  • Deliver skilled interventions including therapeutic activities, self-care training, adaptive equipment recommendations, and environmental modifications.
  • Adjust treatment plans as required and maintain communication with physicians and the care team.
  • Educate patients and caregivers on functional independence, safety, fall prevention, and compensatory strategies.
  • Document patient care electronically in Home Care Home Base (HCHB) in a timely and accurate manner.
  • Collaborate with interdisciplinary professionalsnurses, PTs, SLPs, social workers, and aidesto coordinate care and participate in meetings.
  • Ensure regulatory, policy, and Medicare compliance throughout all clinical work.

Qualifications:

  • Active Occupational Therapist license in good standing.
  • At least one year of recent Home Health Occupational Therapy experience (field experience preferred).
  • Experience with Home Care Home Base (HCHB) and OASIS assessments.
  • Strong clinical, assessment, and time management skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently and adapt to a flexible, field-based caseload.
