Job Description
Make an impact as a Home Health Occupational Therapist, providing patient-centered care to individuals in a home setting. In this contract position, you will empower patients to regain independence, improve safety, and enhance quality of life through targeted occupational therapy interventions. Youll conduct evaluations, design individualized care plans, and educate patients and families, all while working collaboratively with a multidisciplinary team.
Schedule & Assignment Details:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract assignment
- Caseload: 2532 points weekly (57 visits/day, varies by visit type)
- Field-based (patients' homes), with required daily travel
- Utilizes Home Care Home Base (HCHB) for electronic documentation
- OASIS assessments required for Start of Care, Resumption, Recertification, and Discharge
Required Qualifications:
- Active Occupational Therapist (OT) license in the state of practice, in good standing
- At least one year of recent experience as a Home Health OT, including field-based work
- Solid experience with OASIS assessments
- Familiarity with Home Care Home Base (HCHB) is preferred
- Reliable transportation, valid drivers license, and auto insurance
- Excellent time management, clinical assessment, and patient education skills
- Ability to manage an independent field-based schedule
Key Responsibilities:
- Perform comprehensive occupational therapy evaluations, including OASIS
- Develop and implement care plans for ADLs, upper extremity function, and home safety
- Recommend adaptive equipment and train patients in its use
- Document interventions and progress in HCHB in a timely manner
- Coordinate care and share timely updates with the interdisciplinary team
- Educate patients and caregivers to maximize independence and minimize risk
