Job Description

Deliver compassionate care and empower patients to thrive in their own environments as a Home Health Occupational Therapist. This contract opportunity is focused on delivering high-quality occupational therapy services directly in patient homes, centered on improving independence, safety, and activities of daily living.

Schedule & Assignment Details:

  • Monday to Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract
  • Approximately 2532 points per week (typically 57 visits per day)
  • Field-based within an assigned territory; daily travel required (mileage reimbursement included)
  • Utilize Home Care Home Base (HCHB) for documentation; OASIS assessments required for specific care events

Key Responsibilities:

  • Conduct comprehensive occupational therapy evaluationsincluding OASIS assessments
  • Develop and implement individualized plans of care focused on ADLs, IADLs, upper extremity function, cognition, and safety
  • Deliver interventions such as self-care training, adaptive equipment education, energy conservation, and environmental modifications
  • Recommend and help procure durable medical equipment as needed
  • Regularly reassess patient progress, update care plans, and ensure communication with physicians and team members
  • Educate patients and caregivers on functional independence, safety, and compensatory strategies
  • Maintain accurate and timely documentation in HCHB
  • Participate in interdisciplinary case conferences and adhere to agency and Medicare guidelines

Qualifications:

  • Active Occupational Therapist license in good standing
  • At least one year of recent field-based Home Health OT experience
  • OASIS proficiency required; HCHB experience strongly preferred
  • Exceptional clinical, communication, and organization skills
  • Reliable transportation, valid drivers license, and auto insurance
