Provide expert occupational therapy care in patients homes as a Home Health Occupational Therapist (OT) in the Raleigh, NC area. This full-time contract role spans 13 weeks and offers a rewarding opportunity to improve patients daily living skills, independence, and safety through personalized care plans and skilled therapeutic interventions.

Key Responsibilities:
- Conduct comprehensive OT evaluations and OASIS assessments to establish patient needs
- Develop and implement individualized treatment plans targeting ADLs, IADLs, cognition, upper extremity function, and home safety
- Deliver skilled interventions such as therapeutic activities, energy conservation, adaptive equipment training, and environmental modifications
- Recommend appropriate durable medical equipment (DME) and provide training as needed
- Assess patient progress, update treatment plans, and communicate with interdisciplinary teams and physicians
- Educate patients and caregivers on functional independence, fall prevention, and safety strategies
- Complete timely, accurate documentation using the Home Care Home Base (HCHB) system
- Collaborate with nurses, physical therapists, social workers, and home health aides for coordinated care
- Ensure compliance with agency policies and Medicare home health guidelines
Qualifications and Desired Experience:
- Active Occupational Therapist license in good standing for the state of practice
- Minimum one year of recent home health OT experience, preferably with field-based care
- Experience with OASIS and proficiency in HCHB preferred
- Strong clinical assessment and functional training skills
- Reliable transportation, valid drivers license, and auto insurance
- Excellent communication, time management, and organizational abilities
- Ability to work independently with a flexible field-based schedule
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Approximately 57 patient visits per day
