As a home health occupational therapist, youll collaborate with interdisciplinary teams and support adults recovering from conditions like post-stroke effects, joint replacement, COPD, CHF, and more. This travel assignment is a standout opportunity to enhance your professional portfolio as you adapt to different home settings, leverage diverse care approaches, and refine your leadership skills through collaboration with experienced RNs and PTs.

Schedule/Hours:

  • Monday to Friday, approximately 0745-1600
  • No weekend, on-call, or holiday shifts
  • Daily caseload of 6 patients/points

What You'll Do:

  • Perform evaluations and treatments focused on ADLs and adaptive equipment for adult home health clients
  • Collaborate closely with interdisciplinary teams (RN and PT) for holistic patient care
  • Attend daily morning team huddles via phone for coordinated care planning
  • Navigate care through electronic records (EPIC) and benefit from robust onboarding, including EPIC training and preceptor support
  • Drive between patient homesmost travel within 20-30 minutes, with rare longer trips

Qualifications:

  • Bachelors degree (or higher) in Occupational Therapy
  • WI OT license and BLS certification required
  • Minimum of 3 years OT experience (SNF, acute care, and particularly home health backgrounds welcomed)
  • Active drivers license, reliable vehicle, and current auto insurance
  • Comfort with business casual or any-color scrubs
