Job Description

Deliver essential occupational therapy services to adults in a home health setting within Port Charlotte, FL. This contract position offers an opportunity to apply specialized skills supporting patients recovering from surgery, managing chronic conditions, or requiring post-acute care. Key Responsibilities:

  • Conduct comprehensive evaluations using standardized assessment tools and develop individualized Home Exercise Programs.
  • Provide skilled care encompassing activities of daily living (ADLs), cardiac rehabilitation, edema massage, splinting, joint mobilization, and use of modalities such as TENS and ultrasound.
  • Educate patients and families on safety, injury prevention, feeding techniques, and the use of prosthetics/orthotics.
  • Perform accurate and thorough documentation and billing complying with Medicare, Medicare Advantage, and commercial insurance standards.
  • Manage 6-8 home skilled visits daily, ensuring efficient scheduling and communication with the healthcare office.
  • Use digital tools including laptop, MiFi, portable printer, and electronic documentation systems.

Qualifications:

  • Current state license as an Occupational Therapist and valid BLS certification.
  • Minimum of 1 year experience in home health OT; 2 years preferred with ability to practice independently.
  • Familiarity with OASIS assessments preferred.
  • Valid drivers license and reliable vehicle for patient home visits.
  • Compliance with vaccination requirements including COVID-19 and flu shot or mask-wearing policies.

Schedule:

  • Day shift, Monday through Friday, 8:00 AM to 5:00 PM.
