Job Description

A rewarding career opportunity is available for a dedicated Home Health Occupational Therapist (OT) specializing in providing patient-centered care within community settings. This contract position in Pink Hill, NC, offers a chance to enhance patients independence and quality of life by delivering skilled occupational therapy interventions directly in their homes.

Responsibilities include performing comprehensive evaluations using OASIS assessments, developing and implementing individualized care plans focused on activities of daily living (ADLs), cognition, and home safety, and training patients and caregivers on adaptive techniques and equipment. Collaborating closely with an interdisciplinary team, you will document progress in the Home Care Home Base (HCHB) system and ensure compliance with Medicare guidelines and agency policies. Regular travel within an assigned territory with mileage reimbursement is required.

Key Qualifications:

  • Active Occupational Therapist license in good standing for the state of practice
  • Minimum of 1 year recent experience in Home Health Occupational Therapy, with field experience preferred
  • Proficiency in OASIS protocols; familiarity with Home Care Home Base (HCHB) preferred
  • Strong skills in clinical assessment, functional training, and patient education
  • Valid drivers license, reliable transportation, and auto insurance
  • Ability to work independently and manage a flexible schedule
  • Excellent communication and organizational abilities

Schedule and Contract Details:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Caseload approximately 2532 points weekly (57 patient visits daily)
