Job Description

Provide expert occupational therapy services in patients homes as a Home Health Occupational Therapist (OT), focusing on enhancing daily living skills, independence, and safety. This full-time, 13-week contract position requires self-direction and strong organizational skills to manage a field-based caseload effectively.

Key Responsibilities:

  • Conduct comprehensive OT evaluations, including OASIS assessments.
  • Develop and implement individualized care plans targeting ADLs, IADLs, cognition, and home safety.
  • Deliver skilled interventions such as therapeutic activities, energy conservation, adaptive equipment training, and environmental modifications.
  • Recommend and assess the need for durable medical equipment (DME).
  • Monitor patient progress, modify treatment plans, and communicate with interdisciplinary team members and physicians.
  • Educate patients and caregivers on functional independence, fall prevention, and compensatory techniques.
  • Complete accurate documentation using Home Care Home Base (HCHB).
  • Collaborate with nurses, physical therapists, speech-language pathologists, social workers, and home health aides.
  • Participate in case conferences and adhere to regulatory standards and Medicare guidelines.

Qualifications:

  • Active Occupational Therapist (OT) license in good standing for the state of practice.
  • Minimum 1 year of recent Home Health OT experience; field experience preferred.
  • Competency in OASIS assessments is required; proficiency with Home Care Home Base (HCHB) preferred.
  • Reliable transportation with valid drivers license and auto insurance.
  • Excellent communication, time management, and organizational skills.
  • Ability to work independently in a flexible, field-based setting.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Approximately 2532 caseload points per week (57 patient visits daily)
