Job Description
Provide essential therapeutic care as a Home Health Occupational Therapist (OT) delivering personalized treatment in patient residences throughout the Peyton, CO area. This contract role offers the opportunity to apply your expertise independently in a rewarding field-based setting, enhancing patients' ability to perform daily activities and live safely and comfortably in their homes.
Responsibilities:
- Conduct comprehensive occupational therapy evaluations, including OASIS assessments for start of care, resumption, recertification, and discharge.
- Develop and implement tailored plans of care addressing ADLs, IADLs, upper extremity function, cognition, and home safety.
- Deliver skilled interventions such as therapeutic activities, self-care training, energy conservation techniques, adaptive equipment instruction, and environmental modifications.
- Recommend and assess appropriate durable medical equipment (DME) to support patient independence.
- Monitor patient progress, adjust treatment plans accordingly, and communicate updates with physicians and interdisciplinary teams.
- Educate patients and caregivers on functional independence, fall prevention, and compensatory strategies.
- Complete accurate documentation using the Home Care Home Base (HCHB) electronic system.
- Collaborate with nurses, physical therapists, speech-language pathologists, medical social workers, and home health aides to ensure coordinated care.
- Participate in case conferences, interdisciplinary meetings, and ongoing professional development.
- Comply with agency policies and Medicare home health guidelines.
Qualifications:
- Active Occupational Therapist license in good standing.
- Minimum of 1 year recent home health OT experience; field experience preferred.
- Proficiency with OASIS required; experience with HCHB preferred.
- Strong clinical assessment, functional training, and communication skills.
- Reliable transportation, valid driver's license, and auto insurance.
- Ability to work independently and manage a flexible field-based schedule.
Schedule:
- Full-time, Monday through Friday, 8:00 AM 5:00 PM
- Caseload averaging 2532 points per week (57 patient visits daily)
