Job Description

Deliver specialized occupational therapy services in a home health setting, providing compassionate care to adult patients with a variety of medical and rehabilitative needs. This role involves comprehensive evaluation and treatment across multiple conditions including cardiac rehab, orthopedic recovery, neurological disorders, and more. You will conduct thorough patient assessments, develop personalized home exercise programs, and educate patients and families on safety, injury prevention, and daily living techniques. Strong documentation skills and familiarity with billing processes are essential for compliance and continuity of care.

Key Responsibilities:

  • Perform evaluations using standardized assessment tools and develop individualized treatment plans
  • Implement interventions including splinting, TENS, ultrasound, joint mobilization, edema massage, and feeding techniques
  • Provide education on safety, lifting/transfers, and home exercise programs to patients and caregivers
  • Manage 6-8 home visits daily, ensuring timely and accurate documentation utilizing electronic systems
  • Communicate effectively through telephone calls and coordinate care as needed

Qualifications and Experience:

  • Licensed Occupational Therapist with home health experience (minimum 1 year, 2 years preferred)
  • BLS certification required; OASIS training preferred
  • Must be an independent practitioner with strong clinical and interpersonal skills
  • Valid drivers license and reliable vehicle required for home visits
  • COVID-19 vaccination or approved exemption, flu shot or declination with mandatory mask use during work hours
  • Comfortable wearing any solid color scrubs

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM (Day Shift)
