Home Health Occupational Therapist (OT) Newburg, MO

A rewarding opportunity is available for a dedicated Home Health Occupational Therapist (OT) to provide compassionate and skilled occupational therapy services in patients homes. This contract position offers the chance to work independently while making a meaningful difference in patients abilities to perform activities of daily living (ADLs) and enhancing their safety and independence.

Responsibilities:

  • Conduct comprehensive occupational therapy evaluations including OASIS assessments.
  • Develop and implement individualized treatment plans targeting ADLs, instrumental ADLs (IADLs), cognition, upper extremity function, and home safety.
  • Deliver skilled interventions such as therapeutic activities, self-care training, energy conservation techniques, adaptive equipment training, and environmental modifications.
  • Assess and recommend appropriate durable medical equipment (DME).
  • Monitor patient progress and adjust treatment plans accordingly.
  • Educate patients and caregivers on functional independence, fall prevention, and compensatory strategies.
  • Maintain accurate and timely documentation via Home Care Home Base (HCHB).
  • Collaborate with interdisciplinary team members to ensure coordinated care.
  • Participate in case conferences and ongoing professional development.
  • Ensure adherence to agency policies and Medicare home health guidelines.

Qualifications:

  • Current Occupational Therapist license (OT) in good standing.
  • Minimum of 1 year recent experience in Home Health Occupational Therapy; field experience preferred.
  • Proficiency with Home Care Home Base (HCHB) preferred.
  • Experience with OASIS.
  • Strong clinical assessment and functional training skills.
  • Reliable transportation with a valid drivers license and insurance.
  • Ability to work independently with excellent organizational and communication skills.

Schedule & Contract Details:

  • Full-time, 13-week contract
  • MondayFriday, 8:00 AM5:00 PM
  • Caseload: Approximately 2532 points/week (57 patient visits/day)
