Job Description

Deliver essential care and promote independence as a Home Health Occupational Therapist, providing expert therapy services directly in patients homes. Your clinical judgment and compassionate approach will help clients safely achieve greater functional ability and confidence in everyday activities.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM

Qualifications:

  • Current, active Occupational Therapist (OT) license in the state
  • At least 1 year of recent home health OT experience
  • OASIS assessment expertise required
  • Experience with Home Care Home Base (HCHB) preferred
  • Reliable transportation, valid drivers license, and active auto insurance
  • Excellent clinical skills and ability to work independently in the field
  • Strong communication, organization, and time management abilities

Core Responsibilities:

  • Conduct comprehensive OT evaluations, including required OASIS assessments for Start of Care, Recertification, Resumption, and Discharge visits
  • Develop and implement individualized care plans to address ADLs, IADLs, upper extremity abilities, cognition, and home safety
  • Deliver therapeutic interventions, self-care training, adaptive equipment recommendations, and home modifications
  • Complete all documentation in Home Care Home Base (HCHB) accurately and on time
  • Educate patients and their caregivers on injury prevention, compensatory techniques, and safety at home
  • Collaborate closely with an interdisciplinary team of nurses, PTs, SLPs, social workers, and home health aides
  • Attend case conferences and maintain compliance with regulatory and policy standards

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
