Job Description

Deliver essential occupational therapy services directly to patients in their homes, focusing on restoring independence and enhancing daily life. As a Home Health Occupational Therapist, you'll provide individualized care, using your expertise to help clients regain or maintain their ability to perform activities of daily living in the comfort of their own surroundings.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM

Qualifications:

  • Active Occupational Therapist (OT) license, in good standing for the state of practice
  • At least one year of recent home health OT experience; field/home-based experience is highly valued
  • Proficiency using Home Care Home Base (HCHB) strongly preferred
  • Solid working knowledge of OASIS assessments
  • Reliable vehicle, valid drivers license, and current auto insurance
  • Excellent clinical, assessment, and organizational skills
  • Self-motivated with the ability to manage a flexible, field-based schedule

Primary Responsibilities:

  • Perform comprehensive OT evaluations, including Start of Care, Resumption, Recertification, and Discharge OASIS assessments as needed
  • Create and execute treatment plans focusing on ADLs, IADLs, upper extremity rehabilitation, cognitive function, and home safety
  • Deliver skilled interventions such as therapeutic activities, self-care retraining, adaptive equipment use, and environment modifications
  • Educate patients and caregivers on safe, independent living, adaptive techniques, and home modifications to support continued safety
  • Recommend and arrange for appropriate durable medical equipment
  • Maintain timely and accurate documentation using Home Care Home Base (HCHB)
  • Participate in case conferences and collaborate with a multidisciplinary care team to optimize outcomes
