Job Description
Deliver essential occupational therapy services directly to clients in their homes as part of a collaborative healthcare team. This full-time, 13-week contract role focuses on enhancing patient independence and safety, emphasizing interventions that improve activities of daily living (ADLs), upper extremity function, and home safety. The ideal professional thrives in a field-based environment, is adept at comprehensive assessments, and values interdisciplinary collaboration to reach positive patient outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive occupational therapy evaluations, including OASIS assessments as appropriate.
- Develop and implement individualized care plans targeting ADLs, IADLs, cognition, and home safety.
- Deliver skilled interventions such as self-care training, therapeutic activities, adaptive equipment instruction, and environmental modifications.
- Recommend durable medical equipment and educate patients/caregivers on use and safety.
- Monitor patient progress, update care plans, and communicate efficiently with physicians and care team members.
- Maintain thorough and timely documentation in Home Care Home Base (HCHB).
- Participate actively in interdisciplinary meetings, case conferences, and professional development.
- Ensure adherence to agency policies and Medicare home health standards.
Qualifications:
- Active Occupational Therapist (OT) license (state-specific) in good standing.
- At least one year of recent home health OT experience; field-based background preferred.
- OASIS assessment expertise required.
- Familiarity with Home Care Home Base (HCHB) is strongly desired.
- Reliable transportation, valid drivers license, and auto insurance are mandatory.
- Strong time-management, communication, and clinical assessment skills with the ability to work independently.
