Deliver essential occupational therapy services directly to clients in their homes as part of a collaborative healthcare team. This full-time, 13-week contract role focuses on enhancing patient independence and safety, emphasizing interventions that improve activities of daily living (ADLs), upper extremity function, and home safety. Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive occupational therapy evaluations, including OASIS assessments as appropriate.
• Develop and implement individualized care plans targeting ADLs, IADLs, cognition, and home safety.
• Deliver skilled interventions such as self-care training, therapeutic activities, adaptive equipment instruction, and environmental modifications.
• Recommend durable medical equipment and educate patients/caregivers on use and safety.
• Monitor patient progress, update care plans, and communicate efficiently with physicians and care team members.
• Maintain thorough and timely documentation in Home Care Home Base (HCHB).
• Participate actively in interdisciplinary meetings, case conferences, and professional development.
• Ensure adherence to agency policies and Medicare home health standards.

Qualifications:

• Active Occupational Therapist (OT) license (state-specific) in good standing.
• At least one year of recent home health OT experience; field-based background preferred.
• OASIS assessment expertise required.
• Familiarity with Home Care Home Base (HCHB) is strongly desired.
• Reliable transportation, valid drivers license, and auto insurance are mandatory.
• Strong time-management, communication, and clinical assessment skills with the ability to work independently.
