Job Description

Deliver high-quality care directly to patients in their own homes as a Home Health Occupational Therapist (OT). In this contract role, you will empower individuals to improve their ability to perform daily activities, maximize independence, and enhance safety within the comfort of their home. Youll work as an integral part of a multidisciplinary care team, executing comprehensive assessments, crafting individualized plans, and guiding both patients and caregivers toward successful outcomes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • 13-week full-time contract

Required Qualifications:

  • Active Occupational Therapist license in the state of practice, in good standing
  • At least 1 year of recent experience providing Home Health Occupational Therapy
  • OASIS assessment expertise required
  • Proficiency in Home Care Home Base (HCHB) preferred
  • Strong clinical assessment, communication, and organizational skills
  • Reliable transportation, valid drivers license, and current auto insurance
  • Comfortable managing a dynamic, independent field-based caseload

Responsibilities:

  • Complete thorough occupational therapy evaluations, including OASIS documentation as needed
  • Develop, implement, and modify treatment plans centered around ADLs, IADLs, upper extremity function, cognition, and home safety
  • Deliver skilled therapeutic interventions such as self-care training, adaptive equipment education, and environmental modifications
  • Assess and recommend durable medical equipment
  • Monitor patient progress, update plans, and collaborate with physicians and interdisciplinary team members
  • Provide education to patients and caregivers on safety, independence, and adaptive strategies
  • Maintain timely, accurate clinical documentation in HCHB
  • Attend relevant case conferences and interdisciplinary meetings
