Schedule:

• Monday to Friday, around 07451600
• No weekend or on-call hours
• No holiday work required

Responsibilities:

• Deliver high-quality occupational therapy services for a caseload of 6 adult patients per day in home health settings across Racine and Kenosha counties
• Focus on evaluation, ADL training, adaptive equipment recommendations, and post-acute rehabilitation
• Work collaboratively within an interdisciplinary team including RNs and PTs, participating in daily morning huddles
• Accurately document patient care using EPIC EMR (EPIC experience preferred)
• Attend a comprehensive orientation session, including EPIC training and mentorship with a preceptor
• Maintain flexibility for rare, longer-distance home visits as patient needs dictate

Qualifications:

• Minimum of 3 years OT experience (SNF, home health, or acute care preferred)
• Active Wisconsin OT license and BLS certification
• Bachelors degree in occupational therapy (or higher)
• Valid drivers license, reliable vehicle, and current auto insurance
• Strong general OT evaluation and treatment skills
