Schedule:
- Monday to Friday, around 07451600
- No weekend or on-call hours
- No holiday work required
Responsibilities:
- Deliver high-quality occupational therapy services for a caseload of 6 adult patients per day in home health settings across Racine and Kenosha counties
- Focus on evaluation, ADL training, adaptive equipment recommendations, and post-acute rehabilitation
- Work collaboratively within an interdisciplinary team including RNs and PTs, participating in daily morning huddles
- Accurately document patient care using EPIC EMR (EPIC experience preferred)
- Attend a comprehensive orientation session, including EPIC training and mentorship with a preceptor
- Maintain flexibility for rare, longer-distance home visits as patient needs dictate
Qualifications:
- Minimum of 3 years OT experience (SNF, home health, or acute care preferred)
- Active Wisconsin OT license and BLS certification
- Bachelors degree in occupational therapy (or higher)
- Valid drivers license, reliable vehicle, and current auto insurance
- Strong general OT evaluation and treatment skills
