Job Description
Seeking a compassionate and experienced professional to provide high-quality occupational therapy services in a home health setting. This full-time, 13-week contract offers the opportunity to make a meaningful impact by helping patients enhance their independence and safety within their own homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Required Qualifications:
- Active Occupational Therapist (OT) license in the state of practice
- Minimum of 1 year of recent Home Health OT experience
- Experience with OASIS documentation and assessments
- Proficiency in Home Care Home Base (HCHB) preferred
- Reliable transportation, valid drivers license, and auto insurance
- Strong clinical assessment and functional training skills
- Ability to manage a flexible, field-based schedule and work independently
- Excellent communication, organization, and time management skills
Key Responsibilities:
- Perform comprehensive OT evaluations, including OASIS assessments as required
- Develop individualized plans of care to address ADLs, upper extremity function, cognition, and home safety
- Deliver skilled interventions such as self-care training, energy conservation, adaptive equipment education, and therapeutic activities
- Recommend and coordinate DME, adaptive equipment, and environmental modifications for patients
- Monitor patient progress, update treatment plans, and maintain timely clinical documentation in HCHB
- Educate patients and caregivers on functional independence, safety, and compensatory strategies
- Collaborate closely with nurses, physical therapists, and other interdisciplinary team members to ensure coordinated care
- Participate in case conferences, meetings, and ongoing professional development
- Ensure compliance with regulatory standards and home health guidelines
