Seeking a compassionate and experienced professional to provide high-quality occupational therapy services in a home health setting. This full-time, 13-week contract offers the opportunity to make a meaningful impact by helping patients enhance their independence and safety within their own homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Required Qualifications:

• Active Occupational Therapist (OT) license in the state of practice
• Minimum of 1 year of recent Home Health OT experience
• Experience with OASIS documentation and assessments
• Proficiency in Home Care Home Base (HCHB) preferred
• Reliable transportation, valid drivers license, and auto insurance
• Strong clinical assessment and functional training skills
• Ability to manage a flexible, field-based schedule and work independently
• Excellent communication, organization, and time management skills

Key Responsibilities:

• Perform comprehensive OT evaluations, including OASIS assessments as required
• Develop individualized plans of care to address ADLs, upper extremity function, cognition, and home safety
• Deliver skilled interventions such as self-care training, energy conservation, adaptive equipment education, and therapeutic activities
• Recommend and coordinate DME, adaptive equipment, and environmental modifications for patients
• Monitor patient progress, update treatment plans, and maintain timely clinical documentation in HCHB
• Educate patients and caregivers on functional independence, safety, and compensatory strategies
• Collaborate closely with nurses, physical therapists, and other interdisciplinary team members to ensure coordinated care
• Participate in case conferences, meetings, and ongoing professional development
• Ensure compliance with regulatory standards and home health guidelines
